This woman came to the office complaining of a long and asymmetric tip. She underwent rhinoplasty and septoplasty to correct this. Shortening a elongated nose is a very difficult and complex procedure. Dr. Zoumalan performs this on a regular basis. This patient required complex cartilage restructuring to allow her tip to set back to an appropriate length for her face. All these measurements were performed in the office pre-operatively in order to have the optimal tip length (projection) for her face. Also when a tip is shortened, the dorsum (bump) has to be further removed. Otherwise, the bump will appear larger. So this was done as well. In addition, she had asymmetric nasal tip cartilages that had to be made more symmetric. She also suffered from breathing problems, and septoplasty helped her as well.

This means that her tip sits too low for her face. Also her nose was very wide before surgery. Dr. Zoumalan sculpted her nose in a way to still retain her ethnic identity, but allowed her face to be more pleasant and elegant with a rhinoplasty. This patient was exceedingly happy, as one can actually see her improved confidence even in these before and after photos.

Because her reduction was so dramatic, she required considerable middle-third restructuring to ensure that her contours would be smooth. She also had thick skin which made the operation more difficult. You can see the dramatic, yet natural, improvement from all angles.

This is a young Middle Eastern woman who had previous rhinoplasty. She then suffered a fall from a scooter while on vacation in Mexico. Dr. Zoumalan provided months of wound and incision care before he performed functional and comsetic nasal surgery. You can see that even after the incisions healed, the nose was crooked and the tip was asymmetric. Because of the combination of trauma and previous surgery, she had little cartilage to reconstruct the nose, so complex cartilage grafting techniques were used (rib, costal). he entire lower third of her nose had to be re-built because existing cartilage was destroyed. she had to have dermabrasion for the incision later on as well, which improved the appearance of her scars greatly. She is doing well and breathing well now.

This patient had thin skin which made her rhinoplasty more challenging. Thin skin is not forgiving, and any imperfections are easily seen. This patient had a wide tip but she had a septum that was fractured into the nose. Dr. Zoumalan did a reconstructive rhinoplasty to improve her both cosmetically and functionally. She was able to breathe tremendously better after surgery as well.

The after result is expected to last a lifetime, given that the restructuring was done in a way to maintain the tip long-term. She also wanted a narrower tip. The crooked nose is very challenging surgery for rhinoplasty surgeons. Dr. Zoumalan uses every single maneuver available to get noses as straight as humanly possible. This patient had a very crooked nose, presumably due to an injury in the past. She required complete restructuring of the nose to allow for straighter nose. The after photo was taken 2 years after surgery, which shows that her healing went well. These are very challenging cases, and the patient must understand that no surgeon can guarantee a perfectly straight nose due to healing. But Dr. Zoumalan uses every tool available to maximize the chances. You can also see how the profile improved as well.
This is another example of a crooked nose. Straightening of the middle and lower third of the nose is very challenging due to the fact that the structures are comprised of cartilage as well as soft tissue. Sculpting these cartilages has to keep in mind that the cartilage likes to go back to where it was before. Dr. Zoumalan uses special maneuvers to make the cartilage lose this memory and maximize the chances that the cartilages will stay in this new position. The after photo was taken one year after surgery. You can see the dramatic change in the tip.
This woman wanted her bump removed and to have a more elegant tip. This close-up shows how elegant the tip can become after rhinoplasty. She needed more tip length ,so this was performed with a caudal septal extension graft to allow for more tip length, which allowed it to become more narrower. At the same time, the bump was smoothed out. Dr. Zoumalan preferred this straight profile to a scooped one. This is a more natural look than a scooped nose, especially for this patient.
Some patients only want tip rhinoplasty. They only want to have their tip down-sized. This patient had mostly a tip rhinoplasty to sculpt her tip. She had a minor bump that was also shaved, but most of the work was on the tip. Dr. Zoumalan will discuss your goals with you. It may be that you only want your tip improved. This is a shorter surgery with less healing time.
This young lady had rhinoplasty to improve her nasal hump. You can see the improvement also in her tip. She had a lot of cartilage depressions that were also improved with complex grafting.
This young woman had two previous rhinoplasties by other surgeons. The second operation by another surgeon involved the use of rib graft. Dr. Zoumalan performed revision rhinoplasty without the use of rib graft. He was able to find cartilage in her spetum to help restructure her nose. Before surgery, she had a pollybeak deformity, which means that her septum is higher than her nasal bones, which appears unnatural. After surgery, her profile is greatly improved, as are her other views.

But the chin implant balanced everything for him, Males often prefer to get a chin implant at the same time of surgery to give them a stronger presence.
This Asian male had a short nose with a large bump. In the operating room, Dr, Zoumalan found very weak nasal tip cartilages that had to be restructured using a septal extension graft, which is a complex maneuver that extends and elongates the tip. The weak cartilages give the impression that the hump is large, but it is actually the weak tip that makes the bump look bigger. One can see the improvement in his nose while still retaining his ethnic attributes. Dr. Zoumalan and the patient used preoperative image morphing of the profile to decide how the patient's nose would best look after surgery. This is how the plan was decided.
Recovery after rhinoplasty can be very quick. Most patients return to work or school one week after surgery with little signs of surgery.
